Parking:

Our building offers valet parking through the S. Santa Monica Blvd. entrance, though our office does not validate.  The public parking lot on N. Roxbury Dr. between the cross streets of S. Santa Monica Blvd. and N. Santa Monica Blvd. has $2/hr pay stations, and the parking lot next to Rite Aid on N. Bedford Dr. offers the first hour free.

First Appointment:

When coming for your initial evaluation, please arrive fifteen minutes early to allow for unexpected traffic or parking delays, as well as for the new patient paperwork.  The doctor does usually run on time. 

If you would prefer to complete the paperwork in advance, please download and fill out the Medical History and Patient Demographics forms above.  The Privacy Practices is for your information only.

Please bring your medical insurance cards and photo ID.

Cancellation Policy:

Please be advised that there is a 24 hour cancellation policy. Missed appointments or appointments rescheduled less than 24 hours in advance are subject to a $75 fee.
